A Brazilian butt lift (BBL) is a cosmetic procedure that uses your own fat to enhance the shape and size of your buttocks. The goal is to fill out and firm up the gluteal area with the added benefit of eliminating unwanted fat from other parts of the body.

How it works

During BBL surgery, liposuction removes fat from your abdomen, back, hips, thighs or arms — targeted donor sites vary based on individual needs and body type. The fatty tissue is purified and then injected into the buttocks. The results are a more rounded derriere and a resculpted and tightened body.

BBL vs. butt implants

Butt implants are a popular option for permanently enhancing your backside. They differ from Brazilian butt lifts in terms of technique and results, as well as potential side effects such as the implants shifting, inverting or rupturing.

Unlike butt implants (which use silicone) and butt injections (which use dermal fillers), a Brazilian butt lift uses your body's own tissues. Through a process called fat grafting, a surgeon moves fat from parts of your body to your butt.

Who is a good candidate?

People of all shapes and sizes may qualify for a BBL, but if you are very thin or have a high BMI, your doctor may have special recommendations about how to move forward.

Pricing for a Brazilian butt lift can be complex. Various factors may influence the overall cost of the procedure, including:

Complexity of the procedure. The more extensive or intricate the procedure, the higher the cost. For example, additional liposuction or contouring can increase the price.

Anesthesia costs. The type of anesthesia used and the duration of the procedure play a significant role in determining the cost. Longer surgeries or specialized anesthesia methods can lead to higher expenses.2

Facility fees. BBLs performed in outpatient facilities tend to cost less compared to those done in hospitals. However, the choice of facility can also impact the quality of care.3

Quality of care. Opting for bargain BBLs may seem cost-effective but often comes with a greater chance of complications. Investing in a reputable surgeon and facility ensures better outcomes and reduces the likelihood of additional corrective procedures.

Indirect costs. Beyond the surgical fees, there are additional expenses to consider, such as:3

  • BBL pillow (a specialized cushion designed to maintain proper positioning and reduce pressure on your newly enhanced buttocks)
  • Compression garments
  • Lymphatic massage treatments
  • Prescription medications

Combining procedures. Expect a price increase when combining a BBL with other procedures like a mommy makeover, which may also include additional liposuction, breast augmentation or a tummy tuck.

Does Insurance Cover BBL Surgery?

Typically, a Brazilian butt lift is considered cosmetic surgery and not medically necessary, so the procedure and any complications that result from it are generally not covered by medical insurance.

What to Expect: From Preparation to Aftercare

Understanding what to expect before, during and after your procedure can help you feel more confident and prepared. By following key preparation steps and recovery tips, you can ensure a smoother experience and achieve the best possible results.

BBL preparation

Staying well-hydrated and having something to eat before your appointment can prevent feelings of lightheadedness or queasiness during the procedure. Your doctor will give you specific instructions for preparation based on your medical history and the specific procedure. Common recommendations include:

  • Avoiding alcohol for at least 48 hours before your appointment to minimize bruising and swelling
  • Pausing certain medications or supplements, such as NSAIDs or blood thinners, as these can affect clotting and increase bleeding
  • Stopping smoking ahead of time, as smoking can interfere with healing and circulation

Before the procedure, you’ll change into a surgical gown and be asked to lie on your stomach to allow your surgeon easy access to the treatment areas.

BBL procedure

If you're under general anesthesia — which is most common — you’ll be asleep during the procedure. In some cases, local anesthesia with numbing medication may be used, allowing you to remain awake.2

The first part of the procedure focuses on removing fat from areas like your stomach, lower back, hips or thighs. Using a small vacuum-like tube (cannula), your surgeon removes the fat through small incisions, contouring these areas in the process. The removed fat is then purified to ensure only healthy fat cells are used for the next step.

Once the fat is prepared, your surgeon will inject it into your buttocks through three to five small incisions. These injections are strategically placed to enhance the size, shape and symmetry of your buttocks, creating a natural-looking result.

Recovery tips and timing

After receiving the injections, it’s normal to experience mild bruising, redness and pain in the treated areas during the first week. You may notice slight asymmetry or small lumps beneath the skin, but these typically improve as the injected fat settles over time. Be sure to follow your doctor’s post-care instructions. Common recommendations include:

  • Apply an ice pack to the area for 20 minutes at a time, using minimal pressure and avoiding direct contact with the skin.
  • Avoid strenuous exercise and heavy lifting for 24 to 48 hours to reduce swelling and bruising.
  • Take acetaminophen, if needed, to manage any mild discomfort.
  • Use a BBL pillow for several weeks to a month to avoid complications.

Potential Side Effects

In addition to bruising, numbness, swelling and pain, other symptoms may occur. These can include:

  • Air may escape into the space between the lungs and chest wall (pneumothorax)
  • Blockage in a blood vessel caused by a fat particle (fat embolism)
  • Breakdown or death of fat tissue (fat necrosis)
  • Buildup of blood beneath the skin (hematoma)
  • Thick, raised scars (referred to as hypertrophic scars)

Although rare, BBL injections can sometimes lead to complications like infection. It’s important to monitor your recovery and contact your doctor immediately if you experience:

  • "BBL smell" — a musty or slightly sour odor that can occur due to trapped fluids, compression garments and the body's natural healing tissues
  • Dark-colored lesions on gluteal muscle
  • Fever, nausea or dizziness
  • Scabbing or discoloration near the treated area
  • Worsening pain or discomfort that doesn’t improve
  • Yellow or green discharge from the injection sites

The most serious complication is pulmonary fat embolism (PFE), where fat enters the bloodstream. Symptoms include:

  • Confusion
  • Difficulty breathing or breathing fast
  • Fast heart rate (above 100 beats per minute)
  • Fatigue
  • Headaches
  • Red or purple rash on the skin (petechiae)
