What Is Nose Reduction Surgery?

Also called reduction rhinoplasty, nose reduction surgery specifically decreases nose size by carefully removing excess bone and cartilage.2 There are several considerations to think about as you prepare to have a nose job, including whether you are a good candidate for surgery. For a nose reduction, it may be required that you:3

  • Are in good physical and mental health
  • Don’t smoke or are willing to quit
  • Want refinement, not dramatic transformation

Types of reduction rhinoplasty

A reduction rhinoplasty addresses multiple concerns at once, including the bridge, tip and nostrils.2 However, you may not need all of these areas addressed. Instead, your surgeon might recommend one or more of the following targeted procedures:

Alarplasty. Alarplasty, also called nasal base reduction, shapes the nostrils to create a smaller-looking nose. If your primary concern is the appearance of your nostrils’ width or flare, this focused cosmetic procedure offers improvement without the invasiveness of traditional rhinoplasty.4

Nasal tip plasty. Tip rhinoplasty emphasizes reshaping and improving the appearance of the nasal tip. It involves modifying the cartilage and soft tissues of the nasal tip to achieve a nose that is more balanced and harmonious with the rest of the face.5

Nose reduction vs. general rhinoplasty

Reduction rhinoplasty is focused on cosmetic improvements, removing excess tissue to decrease the size.2

General rhinoplasty is cosmetic or medically necessary for functional improvements, like correcting a deviated septum for breathing. It may increase or decrease nose size, often requires grafts or implants and osteotomies (cutting or shaving the bone) and includes reconstruction and augmentation options.3

Factors that may influence costs

There are multiple reasons you may pay more (or less) than the stated cost. Keep these in mind when calculating your total investment:6

  • Anesthesia costs depend on whether you have general anesthesia and vary by the anesthesiologist’s expertise.
  • Combination procedures may include discounts if the rhinoplasty is performed alongside other cosmetic or functional surgeries.
  • Cosmetic surgeons’ fees vary depending on their experience, expertise and geographic location.
  • Diagnostic tests and imaging, like X-rays or CT scans, are often required for preoperative assessment of nasal structure and function.
  • Facility fees cover the cost of the operating room or outpatient surgery facility used for the procedure.
  • Preoperative and postoperative care, including consultations, follow-up visits and additional recovery care, can contribute to the overall cost.
  • The type and complexity of plastic surgery, such as alarplasty or nasal tip-plasty, can impact the price you will pay.

Does Insurance Cover Reduction Rhinoplasty?

Nose reduction rhinoplasty is typically not covered by health insurance when done for cosmetic reasons. However, if the procedure is medically necessary, such as to alleviate breathing difficulties, repair damage resulting from an injury, fix a congenital defect or prepare for a sinus surgery, your insurance may pay for some or all of the procedure.7

If your healthcare provider recommends nose reduction for medical reasons, it’s important to review your insurance policy carefully. Understanding what’s covered and the medical documentation required can help you avoid surprises during the pre-authorization process.7

How Reduction Rhinoplasty Works

There are two main approaches to rhinoplasty for reductions, along with several specific procedures. Understanding these options before your consultation will help you collaborate with your surgeon to create a tailored surgical plan that addresses your concerns and sets realistic expectations for the outcome.3

Here’s a look at open rhinoplasty vs. closed rhinoplasty procedures.

Open rhinoplasty approach

Open rhinoplasty provides the most comprehensive access for complex nasal reductions. Your surgeon can see and modify every structure with maximum precision.3

During open rhinoplasty, your surgeon will make a small incision across the columella (the tissue strip between your nostrils) and lift the nasal skin upward. This will expose the entire nasal framework, allowing direct visualization of all cartilage and bone structures. The surgeon will then reshape, remove or add tissue with precise control before repositioning the skin and closing the incision.3

Closed rhinoplasty approach

Closed or scarless rhinoplasty works entirely through internal incisions. This technique works well for mild to moderate cases, such as dorsal hump removal and subtle tip changes.3

During the surgery, your surgeon will create small incisions inside each nostril and reshape cartilage and bone using specialized instruments. Compared to an open rhinoplasty, a closed approach limits the surgeon’s visual access to the nose structure. However, because the surgery is less invasive, there will be less scarring.3

Nose reduction recovery and timelines

Recovering from nose reduction rhinoplasty is a gradual process that takes several months. Here's a breakdown of what you may expect:8

Week one

Right after surgery, you'll have a splint or bandage to support your nose's new shape during the first stages of healing. You’ll likely experience some bruising and swelling around your nose and under your eyes, which usually improves significantly within the first week.

Week two

Your surgeon will remove the splint, packing, and any external bandages at your first follow-up visit after surgery. You may feel like returning to most normal activities, but it's typically recommended to continue avoiding strenuous activity.

Month one

While the most noticeable swelling and bruising fade within two to three weeks, some internal swelling can persist for several months. Continue to closely follow your surgeon's instructions to ensure a speedy recovery.

Months two to three

During this phase of recovery, you'll notice significant improvements in the shape and size of your nose. It will begin to feel more natural with swelling confined to the tip, which is the last area to heal.

Months six and beyond

It can take up to a year for swelling in the tip of your nose to completely resolve. The final shape and contour of your new nose will become increasingly evident during this stage of recovery.

Potential complications

While rhinoplasty is generally safe, knowing which complications you might face can help you recognize them early. Most issues resolve with proper care, but early detection makes a significant difference in outcomes:3

  • Excessive bleeding
  • Septal hematoma (blood clot)
  • Severe infection/pain
